The Medicare Solutions Counselor will serve as a Medicare resource for the Advocate Aurora Health s site of care its patients, clinicians and team members and surrounding service area senior residents.
A. Complete State Health Insurance Assistance Program (SHIP) certification which includes state fingerprint background check.
B. Once certified as a SHIP Counselor, the team member will support beneficiaries as they enroll in Medicare and Medicare insurance products and assist them with all programs related to Medicare.
C. Conduct direct-to-patient outreach to soon to be Medicare-eligible individuals as needed. Provide one-on-one appointments to those new to Medicare and existing Medicare beneficiaries, families, caregivers, AAH employees and the community to explain/educate/enroll in Medicare and Medicare related products and assist with all programs relating to Medicare.
D. Conduct individual health insurance counseling sessions affording complete confidentiality to the client without conflict of interest.
E. Gather information from appropriate sources needed to help consumers solve Medicare related issues. Interact with local, state, and federal agencies to research consumer s questions. Connect patients to additional resources including health plans and/or broker resources, as applicable.
F. Positively interact and feel comfortable working with diverse populations. Clearly communicate with other counselors, acting as a team.
G. Host and coordinate educational seminars and updates covering senior related Medicare topics.
H. Facilitate and coordinate senior health and wellness programming to align with identified health needs. Assess and evaluate additional programs and external locations and make recommendations for improvement outside of AAH. Attend local Senior Health Fairs and events.
I. Complete client contact reporting for each counseling session.
J. Report all functions and activities monthly.
